Please note that it is the policy of the Union1 District NOT to include any information on this website that includes date, times, and places for Girl Scout events for safety reasons. Therefore, links to Registration Forms for District Events CANNOT be found on the website. Information regarding District Events is contained in the Monthly Newsletters and in emails from the committees. You may also find forms on the Union 1 Girls Scout Big Tent Group under the Forms page.

Please keep the emails & newsletters and refer to them as needed.

Registration Forms

Girl Registration Form - Use this form to register any girl into the Girl Scout program.
Adult Registration Form - Use this form to register as a Leader, Co-leader, Team Member or any other Adult Volunteer Position within Girl Scouts.
Dues Summary - Use this form to include with any payments when submitting registration forms.
Registration Assistance - Use this form when requesting assistance with any registration. 
Change of Member Information - Use this form when updating information such as address, personal information or troop number.

Financial Forms

Bank Account Procedures - This document outlines the procedures for setting up troop bank accounts.
Semi-Annual Troop Report - This report is submitted twice each year on January 15 and June 15.  If you are applying for the Outstanding Troop Award, the 2nd due date is April 30 - not June 15.
Financial Assistance Form - Use this form when requesting assistance for those in financial need.
Troop Money Earning Activity - Use this form to request approval on any money earning activity.

Troop Management

New Leader Checklist - This document can be useful for leaders starting new troops.
Leader Training Form - Use this form to document training dates. Leader awards are based on completion of specific training.
Girl and Adult Health History - Use this form for overnight field trips or in any physically demanding activities.
Outstanding Troop Award - Use this form to track your troop's activities throughout the year to qualify for this award.
Girl Scout Safety Guidelines - This document contains helpful information to keep our Girl Scouts safe.

Additional Forms

Permission Form
Anytime an activity takes place outside of your normal meeting time and/or place, a Permission Form needs to be given to the parents and also a troop permission form submitted to Darlene Foreman at least 1 month prior to the activity. When the form is complete, you can email it directly from Word or save it to your computer and forward it on to Darlene. 

If you are unsure of the required training for leaders when taking a field trip that includes high risk activities please click here.  Additional information is also available from Council's website in the section called Safety Activity Checkpoints click here. You will need to click on a specific activity to read about the requirements of that activity.

Troop Trip Application
For each trip, submit 1 copy of the Troop Trip Application along with the itinerary and a roster of all girls/adults attending the trip to GSHNC Customer Service Department 30 days prior to the event.  Darlene Foreman will need the permission slip only.

Community Service Form
Required for every community service project, including those that are Service Unit sponsored; submit a copy of the service project form to Council prior to or after the event. This form should be forwarded to the current Membership Specialist at Council. (Look under the Community Service tab for information on projects for you troop!)

Badge Tracking Program - a third party item that was developed to help you track the badges earned by your girls.

Program Aide Self Paced Course
For older scouts, Cadettes, Seniors and Ambassadors looking to assist during Union 1 Day Camp, completeion of this course is currenlty required. Due to the recent curriculum change at all levels this program is also being evaluated and updated to align with the current curriculum. For questions regarding this training please contact Kathleen H.
